Submitting this form means that you are making a personal commitment to make an annual donation of $400 ($100 at each quarterly meeting), to local charities serving those living in the Adirondack Foothills area. Even if the charity chosen is not your first choice, you are committing to donate at each meeting. If you are not able to attend the quarterly meeting, you should make arrangements to submit your check through another member or mail your check to PO Box 1 in Johnstown immediately after the meeting.
